Program Applications

If you are a young person between the ages of 5 and 17 and are interested in being a participant in any of the One to One programs, please call or email us to set up an interview

If you are a teacher or school adminstrator and know of a child who could benefit from any of the One to One programs, you are welcome to fill out a Teacher/Counselor/Administrator referral form on behalf of the child.

If you are a parent and wish to enroll your child in our programs, please fill out a Parent Application Form .  Please contact us if you need a Parent Application Form in Spanish.

If you are interested in becomming a volunteer Mentor of Study Buddy, click "Become a Mentor" button at the top right hand side of this page to download a form.

If you prefer, One to One can send you a printed application.  Please call us at (970) 728-0885 or email onetoone@telluridecolorado.net with your name and mailing address.
